How can an LLC be formed online, and can a bank account be opened after forming an LLC?

The answer is yes and no. Today virtually all states do have some form of online incorporation—whether it be an LLC, a corporation or some other entity. Go to the secretary of state website you want to incorporate in (which state can be important in the future—say if you want to raise money) and follow the online process. Getting a bank account online is a little more difficult. With the today’s money laundering laws, you typically have to go to a bank, present your articles of organization and sign various forms. There are things like PayPal which are totally online but even they require you to link to a traditional bank account.
